Registration Record

The Registration record is created when a transplant patient is added to the ISHLT Transplant Registry. There are 5 pages to the Registration record: Patient Registration, Pre-Tx, Transplant, Donor and Discharge.   After entering information on each page, click the button at the bottom of the page to save your changes.

From any page on this record, you may:

• Return to the list of records by clicking the Return to List button.

• Create additional follow-up records by clicking the Add Follow-up button.  

Note: annual follow-up records are automatically generated on the transplant anniversary date.

• View existing follow-up records by clicking the Follow-up History button.

• View and print the registration record by clicking the Printable Form button.
