link at the top of the ISHLT Transplant Registry main page, the Add a New Patient page will display. Complete all the required fields (any field that is not noted as "optional"), and any other available information.
After clicking the link at the top of the ISHLT Transplant Registry main page, the Add a New Patient page will display. Complete all the required fields (any field that is not noted as "optional"), and any other available information.
Then click the 
button at the bottom of the page.
A message will display indicating that you successfully added the patient to the registry.
Click the Continue button to complete the remaining pages (Pre-Tx, Transplant and Donor Discharge) of the record. Click here for descriptions of each field on the Patient Registration page.
